To qualify, you must be enrolled in Medicare Parts A and B with Medicare as your primary insurance. If you are not enrolled in Medicare, please contact our Care Team to learn more.

To be eligible, you must be enrolled in Medicare Parts A and B with Medicare as your primary insurance. You are not eligible if your primary insurance is Medicare Advantage or another Medicare health plan (including Special Needs Plans and the Program of All Inclusive Care for the Elderly). You are not eligible if enrolled in Medicare hospice benefit or if you are a long-term nursing home resident (defined as a nursing home stay of 4 months or longer and not covered under the Medicare skilled nursing facility benefit).

If you are eligible for GUIDE, you will be fully covered by Medicare for all services you receive through the program.
The CMS GUIDE Model may include comprehensive dementia assessments, personalized care planning, medication management, caregiver education, care coordination, 24/7 support, and referrals to community resources. Services are provided based on each patient's individual needs and program eligibility.
The CMS GUIDE Model is a Medicare initiative. Eligible beneficiaries enrolled in Medicare Parts A and B may receive GUIDE services covered under Medicare. Coverage depends on individual eligibility and CMS program requirements.
